Fall Intervention

Each year, one in every three adults age 65 or older takes a fall. Two-thirds of those who fall will fall again. Fractures from such falls are a leading cause of disability and often result in nursing home admissions. Thankfully, Partners In Care has jumped in with both feet to put an end to unnecessary falls.

Our Fall Intervention Program (FIP) helps individuals who have recently fallen, fall frequently or are at risk of falling. We send an interdisciplinary team consisting of a registered nurse, a physical therapist and an occupational therapist to identify the root causes behind the falls. Together, they provide:

  • A complete home safety assessment
  • A medication review to evaluate risk factors
  • An individualized exercise program designed to increase strength and balance

Best of all, the Fall Intervention Program assures that you won’t take the fall financially either. With appropriate diagnoses and a physician’s referral, Medicare and most insurance plans cover all of our services. Anyone can refer a patient to the program, even if he or she has had a fall that didn’t result in injury.
