Transitions is a no-cost, non-clinical case management program for those living with a life limiting illness. Our Transitions coordinators will work with you and your family to assess for individual needs, and identify resources. We help you clarify your goals and explore the kind of assistance you might need to bring more quality of life to your everyday routine.

A significant part of the Transitions program is connecting clients with a trained volunteer. Volunteers can provide companionship, emotional support, and more.

Transitions Services Offered by Partners In Care

Our Transitions coordinators recognize that the type of support and resources needed will be different for each individual. Our goal is to meet individuals where they are, and continually seek new ways to support you through your journey.

This program is for people with a fragile medical condition who need:

  • An extra layer of support.
  • Help navigating clinical and community resources.
  • Information to help make informed healthcare decisions and advance care planning.

We welcome referrals from family, friends, physicians, nurses, social workers, and others concerned about the well-being of their loved ones and patients. Transitions brings support, resources, and information into your own home, care facility, or adult foster care home throughout Central Oregon.

Partners In Care continues to serve the needs of our patients and their family members while following all of the guidelines outlined by the Oregon Health Authority and the Centers for Disease Control and Prevention regarding the COVID-19 coronavirus.   • We are still admitting new patients to both Home Health and Hospice.
  • Prior to every in-person visit, all patients receive a risk assessment for COVID-19.
  • Visitors to Hospice House are being carefully screened prior to visiting patients.
  • Our professional caregivers are exercising diligence in all aspects of caring for themselves and others.
