Under the direction of the patient’s physician, we provide skilled services 24 hours a day, 7 days a week, including:
Areas of Excellence
Daily Telemonitoring services provide peace of mind for the patient and family.
Our staff is certified to administer IV therapies, such as antibiotics and hydration.
A multidisciplinary team approach to our Fall Intervention Program takes steps to keep our patients on their feet.
Our comprehensive wound, ostomy and continence care management uses the latest techniques and products in care management.
We offer support, education and case management to help patients navigate diverse community services, including our own Feet Retreat, a foot care clinic that is available to the community as well as home health patients.
Registered Dieticians assist with diabetic and special-need diets.
Our Medical Social Workers help find community resources to maintain each patient’s independence and support their families.
To qualify for home health care, a patient must be:

Be “homebound” or have a normal inability to leave home without considerable assistance

Be under the care of a physician

Require the skills of a nurse on an intermittent basis and/or require the skills of physical therapy, occupational therapy and/or speech therapy
Home Health is a Medicare benefit
