Hospice
Selecting hospice care is an important choice in the cycle of life. Hospice helps people facing end-of-life decisions choose how their final days will be spent.
Since 1979, Partners In Care has been providing exceptional end-of-life care for the terminally ill and their families. We offer an array of compassionate services and therapies through our team of professionals to comfort and support the patient, their family, and friends.
Hospice is unique. It neither hastens nor postpones death. It is about living one day at a time while emphasizing dignity and quality of life. While hospice treats the patient’s pain and symptoms, we also address the concerns of the family and caregiver. We strive to meet the physical, psychological, social, and spiritual needs of the entire family, remaining sensitive to the values and beliefs of each individual.
Partners Hospice provides a comprehensive team-oriented program. Services are brought to the location of the patient, including their private home, care facility, assisted living facility or our own Hospice House. Medication, supplies, and equipment pertaining to the diagnosis will also be provided at no cost.
Hospice is a Medicare benefit

Your Hospice Team will include
- The patient's primary care physician
- Partners Hospice Medical Director to coordinate care
- Registered Nurses who provide pain control, management of symptoms, and caregiver training for patient care
- Social Workers who provide coordination of resources and other assistance as needed
- A Chaplain for spiritual support if requested
- Home Health Aides who can provide personal care, hygiene, and other daily tasks
- Trained Volunteers who can help with a variety of needs and services
- Complimentary services that increase comfort such as massage, acupuncture, harpist and physical and occupational therapy
- Rehab services as needed for comfort
