Adapting to the changes that accompany a chronic or serious illness can be difficult, frustrating, and sometimes confusing. It can be challenging to keep one’s independence while also learning to rely more on others.

Transitions offers compassionate support, resource linkage, and enhanced volunteer service to individuals and families. There is never a charge for any person to be on Transitions and it is available wherever you reside – in your own home, an assisted living facility, or a foster care home.
