Partners In Care Veteran Support Services

Veterans hold a very special place in the hearts of the team at Partners In Care. Veterans have devoted themselves in service to our country, and we are honored to serve them with the best possible care.

Our Veterans Outreach program is unique among hospice organizations in the region. We are one of only three hospice organizations in the state of Oregon to be recognized by the National Alliance for Home Care as the highest level of ‘We Honor Veteran’ programs due to the exceptional and tailored care provided to veterans on our services and in the community. Our team of support specialists is committed to providing specialized care for each of our military member patients and their families. We understand the unique health care and mental health needs and challenges of military veterans because some of our staff and our veteran volunteers served in the armed forces and they continue to serve and find ways to provide assistance to the veteran population in our hospice, home health, palliative care, and transitions programs.

Special Veteran Services

In addition to the many Partners in Care medical and support services we provide to all patients, we work with veterans and their families to ensure they receive the benefits they qualify for regarding their military service. Our Veterans Outreach Coordinator works directly with the Department of Veterans Affairs, both state and local, to find answers to your questions, help navigate serious illness, and ensure you or your veteran are receiving the best possible care. We also connect families with other veteran programs that can enrich their bond, memories, and relationship with their loved one.

We Honor Veterans Program

Partners in Care is a nationally certified, five-star member of the National Hospice and Palliative Care Organization We Honor Veterans Program. We are dedicated to providing the highest level of Veteran-centric services that address their needs through our Vet-to-Volunteer program, recognition ceremonies, and help navigating VA benefits.

How does Partners In Care support veterans receiving hospice or palliative care in Central Oregon?
Partners In Care proudly honors our military veterans with specialized hospice and palliative care designed to meet their specific physical, emotional, and spiritual needs. As a We Honor Veterans partner, we provide compassionate support, recognition ceremonies, and coordinated care tailored to each veteran’s life experiences and military service.
What’s the difference between hospice and palliative care for veterans?
Hospice care is for veterans facing a terminal illness with a life expectancy of six months or less and who are no longer receiving curative treatment. Palliative care, however, is available at any stage of a serious illness—providing relief from symptoms and stress while veterans continue with treatments or therapies.

Is home health care available for veterans through Partners In Care?
Absolutely. Home health care services help veterans recover from illness, surgery, or injury in the comfort of their own homes. Our skilled team offers nursing visits, physical therapy, occupational therapy, and chronic disease management. We collaborate with the VA when possible to coordinate covered care for eligible veterans.

Can veterans receive hospice or palliative care at home?
Yes. Partners In Care provides both hospice and palliative care in the veteran’s home, a long-term care facility, or wherever they feel most comfortable. Our goal is to respect each veteran’s preferences, promote dignity, and enhance quality of life.

We are the only local, nonprofit provider in the region with a commitment to veteran-centered end-of-life care. As a Level Four We Honor Veterans partner, our staff is specially trained to address the impact of military service, trauma, and veteran-specific health concerns. What hospice services are available for veterans in Central Oregon?
Veterans receiving hospice care through Partners In Care have access to pain and symptom management, medication support, emotional and spiritual counseling, medical equipment delivery, and 24/7 on-call nursing care. Our team includes social workers, chaplains, nurses, and volunteers trained to understand the unique needs of those who have served.
Are there any special benefits or programs for veterans in hospice or palliative care?
Yes. Veterans may qualify for benefits through the VA (U.S. Department of Veterans Affairs), including full coverage of hospice services under the VA health system. Our team helps veterans and family members navigate VA benefits, eligibility, and enrollment. We also offer military recognition, commemorative pinning ceremonies, and support groups tailored to veterans and their loved ones.

How do I know if my loved one qualifies for hospice care?
Hospice eligibility is typically based on a physician’s prognosis of six months or less to live if the illness runs its natural course.
