Embark on a journey that explores the intricate tapestry of social determinants of health and the transformative power of trauma-informed practices. Through this exploration, we will unveil the strategies to not only nurture health but also instill a sense of hope within individuals and communities. Through engaging narratives, thought-provoking insights, and practical takeaways, this keynote session aims to inspire a collective commitment to compassionate care. We will discuss how by weaving empathy into the fabric of healthcare, we can amplify the impact of our efforts, transform challenges into opportunities, and create a healthcare ecosystem that thrives on compassion and resilience. Together, we will uncover innovative strategies to build a more compassionate and thriving healthcare ecosystem in Central Oregon. Dr. Qwynn Galloway-Salazar is the founder of In Their Honor, an Army Veteran, End-of-Life Doula, and Educator.

“The Case for Community-Based Palliative Care”
– Steph Barss, NP, Palliative Care Specialist
10:15 AM – 11:00 AM

Steph is a board-certified family nurse practitioner and psychiatric mental health nurse practitioner, with a postgraduate certificate in psychedelic harm reduction and integration. She is a governor appointed member of the Oregon Psilocybin Advisory Board. Her work with the Partners In Care Palliative Care team is focused on supporting people to find meaning, clarify values, identify goals, and maximize quality of life in the setting of advanced illness. Steph’s perspective on supporting medically fragile individuals and their families will help medical and care professionals discover a patient’s optimal goals of care in a community-based setting.

“A Healing Place: The Medical Recuperative Model”
– Cynthia Maree, MD, St. Charles Health System
11:15 AM – 12:15 PM

Besides a shelter and the street, where can someone without housing recover from surgery? Identifying a third option for unhoused patients for discharge is paramount in these times of increased poor discharge options. Without a safe, clean environment to heal, the rehospitalization rate of unhoused medically fragile individuals will continue to rise in spite of payor incentivization. Join Dr. Cynthia Maree as you learn to understand the current local discharge options available for unhoused patients; identify ways to partner with stakeholders to develop safe and dignified housing for medically fragile people; and navigate the upcoming developments in medical respite for our local community: staggered sites, brick and mortar, and other options
